If you have any questions about how to submit an electronic claim, please follow these instructions

1. Go to our website http://www.firstcontdental.com
2. At the bottom of the screen, click “Dentists Login Here”. You will be redirected to our secure “I-transact Portal”.
3. If you have no already created a user account, please click on “Click here to create a new user”.
4. Providers, please select “Provider” from the drop down to create an account.
5. You will be prompted to enter your last name, date of birth, NPI number, and a provider number (*This number is a unique number assigned to each provider in our system; please call our Provider Hotline, and we will provide you the number.)
6. Username can be anything except symbol characters (ex. *,%,$,@,and dashes). Password must be at least 7 characters. Please keep your username and password in a safe place.
7. Last, click Create Account (This will take you back to the login screen.)
8. After creating your account, please enter your username and password to login.
9. On the left side of your screen, click “Submit a Claim"
10. Then search for your patient by Last Name, First Name, or DOB.
11. Enter the service date, procedure code, and the amount of your claim on a per line basis. If necessary, you may also submit an attachment, or a radiograph, with your claim.
12. To finish, click on “Submit Claim” at the bottom of the form.
13. To check claim status, click on “Claim Status” on the left side of your screen. Enter the required information to find the claim. Once the claim is found, results will indicate “Pending” or “Paying” for your claim.
