Member Eligibility Verification
Dental providers should request and verify current eligibility information from all members by asking the Member to produce the Health Plan Identification Card, 3087, and/or 2085 (Foster Care) issued for the term that services are being covered. Members are pre-informed they should share eligibility information with all providers.
