Member Eligibility Verification

Dental providers should request and verify current eligibility information from all members by asking the Member to produce the Health Plan Identification Card, 3087, and/or 2085 (Foster Care) issued for the term that services are being covered. Members are pre-informed they should share eligibility information with all providers.

If clients have lost their identification or forgotten to bring it to appointments, providers may verify their eligibility by:

  1. Contacting FCL’s Member Services Line - 1-866-708-8795
  2. Log-in to FCL website: (http://www.firstcontdental.com/index.htm)
  3. Monthly Provider roster (capitation providers only)
  4. Copies of health plan ID Cards
