Health Passport collects key Member health information and stores it in a secure website. Medical Consenters, health care providers, DFPS caseworkers and some people who work for STAR Health may have access to the information. - Health Passport Forms - PDF versions of the forms available via the Health Passport available for print/download. These forms include all THSteps assessment forms that are required for Health Passport. Providers may complete the forms electronically within Health Passport or Print them off here, complete, and then mail or fax them in using the cover letter also in this resources section.

Features
Once you have accessed the Health Passport you will be able to see the following information:
- Medications - The Health Passport presents a summary of medications. With a single click you can access more detail, including the name of the prescription, the prescribing clinician, date filled, dosage, quantity, dosage instructions, and formulary information. This aggregation of medication information can significantly reduce incidents of drug-drug, drug-allergy, and drug-food interactions. *
- Visit History - With the Health Passport, you can view information regarding visits for which claims have been paid or denied, including the description of the service provided, the provider’s name, diagnosis, and the service date. *
- Vital Signs - You can review vital signs charted by a provider. This can include weight, height, blood pressure, and temperature.*
- Lab Results - Providers typically only have lab results for tests that they ordered; medical consenters and case workers may never see them. However, by using the Health Passport you have access to all collected lab results.
- Immunizations - This section displays a list of a STAR Health Member’s immunizations collected from Immtrac, the state immunization registry.*
- Allergies - You can review existing allergies that have been noted in the Passport by providers. You will also be able to see if there are any allergy and medication interactions.
- Texas Health Steps (THSteps/EPSDT) - You will be able to see electronic documentation of well-child screenings and forms. Some providers may also choose to add additional information outside of these standard childhood screening forms into the system. In those cases, you may be able to view more detailed medical and behavior health information that is relevant to the child’s care.
Please note that the Health Passport is not an Electronic Medical Record. It will NOT contain ALL immunizations, visits, allergies, labs, or medications, but rather only those that Superior is made aware of via claims and/or state-supplied and provider-supplied data. Please use this tool as a general overview of a child’s past and present care and conditions. HOWEVER, THE HEALTH PASSPORT SHOULD NOT REPLACE OR OVERIDE RECORDS OBTAINED DIRECTLY FROM STATE AGENCIES OR PROVIDERS.
